The short answer is that an SMP session typically takes between 2 and 5 hours, although a particularly large treatment area or full-head SMP can take up to 6 hours.

There isn’t one set treatment time because every scalp is different. The extent of your hair loss, the size of your head, the area being treated and even which SMP session you’re having can all affect how long your appointment takes.

How Long Does an SMP Session Take?

As a general guide, you can expect:

  • Smaller treatment areas: Around 2 to 3 hours
  • Larger areas or advanced hair loss: Around 3 to 4 hours
  • Full-head SMP: Around 4 to 6 hours
  • SMP top-up: Often around 1.5 to 2 hours

These are averages rather than fixed appointment times. Some treatments will be quicker, while others may require considerably more work.

What Determines How Long Your SMP Treatment Takes?

The biggest factor is simply how much of your scalp needs to be treated.

Someone experiencing thinning hair through a relatively small area on the top of their head will naturally require less treatment time than someone receiving SMP across their entire scalp.

Other factors include:

  • The extent and pattern of your hair loss
  • The size of your scalp
  • The amount of density required
  • How much existing hair you have
  • The amount of blending required
  • Whether you’re having your first, second or third session
  • Whether we’re treating or disguising any scars

This is why it’s difficult for any SMP practitioner to give an exact treatment time without first seeing your scalp.

How Long Does Full-Head SMP Take?

A full head of Scalp Micropigmentation is one of the largest treatments we carry out and can take approximately 4 to 6 hours per session.

This can involve treating the crown and top of the scalp while creating a new hairline and carefully blending the SMP through the sides, back and towards the neck.

It’s a significant amount of work, and creating a natural result requires thousands of carefully placed impressions.

However, something that often surprises clients is that each SMP session doesn’t necessarily take the same amount of time.

Why Can Your Second SMP Session Take Longer?

You might assume that your first SMP session would always be the longest, but that’s often not the case.

At Creative Scalps, we take a conservative approach during the first session. We begin with a lighter pigment and create the foundation of the treatment, leaving slightly more space between individual impressions.

During your second session, we work back over that foundation and begin increasing the density.

For example, a full-head treatment might look something like this:

  • Session One: Around 4 hours
  • Session Two: Around 5 hours
  • Session Three: Around 4 to 5 hours

The second session can take longer because we’re placing additional impressions between the first layer, increasing density and carefully developing the blend.

Around the hairline, edges and other transition areas, the spacing is adjusted to create a softer fade into your natural hair.

It’s not simply a case of adding more dots. We’re constantly adjusting density and spacing depending on the area of the scalp we’re working on.

How Long Does SMP Take for Advanced Hair Loss?

For someone experiencing more advanced hair loss, such as a Norwood 7, a typical first session might take approximately 3 to 3.5 hours.

The Norwood Scale is commonly used to describe different stages of male pattern hair loss, with Norwood 7 representing one of the most advanced patterns.

For this type of treatment, you might expect:

  • Session One: Approximately 3 to 3.5 hours
  • Session Two: Approximately 3.5 to 4 hours
  • Session Three: Approximately 3 hours

Again, these times can vary considerably. A larger scalp naturally means there’s more area to cover, while the amount of existing hair and blending required will also influence the treatment time.

What About a Norwood 5 SMP Treatment?

Someone with a Norwood 5 pattern will typically have a smaller area to treat than someone with a Norwood 7.

A first SMP session for this type of hair loss might take around 2.5 to 3 hours.

However, we don’t necessarily stop the treatment exactly where the visible hair loss ends.

At Creative Scalps, we’ll often blend the SMP into the surrounding sides and back. This helps create a seamless transition into your natural hair and can also help accommodate future thinning or recession.

That additional blending takes time, but it’s an important part of creating a treatment that looks natural rather than simply filling the obvious bald area.

How Long Does an SMP Top-Up Take?

If you’ve already completed your initial SMP treatment and return in the future for a top-up or refresh, your appointment will usually be considerably shorter.

Rather than recreating the entire treatment, we’re typically restoring areas where the pigment has softened over time and bringing the overall appearance back to life.

A treatment that originally required around three to four hours per session may only require around 1.5 to 2 hours for a top-up, although this will depend on how much refreshing is required.

How Many SMP Sessions Will I Need?

Most clients require approximately three SMP sessions to achieve their finished result.

The process usually works like this:

  • Session One: Establish the foundation using a lighter shade and lower density.
  • Session Two: Build density, improve the blend and introduce additional impressions.
  • Session Three: Assess the healed result and refine any areas that need additional work.

Some clients may only require two sessions, while others may benefit from a fourth. Your treatment plan will depend on your skin, hair loss and desired result.

How Long Does the Entire SMP Process Take?

Although the actual treatment time is measured in hours, your complete SMP journey takes longer because your scalp needs time to heal between sessions.

Your appointments are usually spaced several weeks apart, giving your skin time to recover and allowing us to see how the pigment has settled before adding the next layer.

This is an important part of the process. SMP isn’t something we want to rush. Building the treatment gradually gives us much greater control over the pigment, density, hairline and overall finish.

Is a Longer SMP Session Better?

Not necessarily. The amount of time someone spends performing SMP isn’t automatically an indication of quality. Experienced artists naturally work at different speeds and may use different techniques.

What matters is the result.

A good SMP practitioner should take enough time to carefully consider your hairline, pigment, density, impression spacing and blend without rushing the procedure simply to finish within a predetermined appointment time.

Equally, taking twice as long doesn’t automatically mean someone is twice as good. Experience, technique and the quality of the finished work are far more important than the number of hours spent in the chair.

If you’re booking an SMP treatment, it’s sensible to allow several hours for each appointment rather than trying to squeeze your treatment into a busy day.

As a rough guide, most clients should expect each session to last somewhere between 2 and 5 hours, while particularly extensive full-head treatments can take approximately 4 to 6 hours.

The most important thing to remember is that your treatment shouldn’t be dictated by the clock.
